Declare a médecin traitant

What is it
Naming one doctor as your regular practitioner with the Assurance Maladie, which places you in the parcours de soins coordonnés. A mutuelle, or complémentaire santé, is a separate thing: cover you take out yourself to top up what the Assurance Maladie reimburses.
Do I need it
From age 16, a declared médecin traitant is what gives you the highest level of reimbursement. Under 16 no declaration is needed and reimbursement is not reduced.
When
Once your health insurance rights are open and a doctor has agreed to take you on.
Where · mixed
At the doctor's surgery, where with your agreement they send the declaration using your Carte Vitale, or by posting form S3704 to your health insurance fund.

- You can check the name recorded from the "Mes informations" section of your compte ameli.
- You can change médecin traitant later without conditions and without giving a reason, by making a new declaration; ameli states the change takes effect immediately.
